Implants and Fillers for Facial Feminization
Facial implants and fillers are common procedures for transgender women seeking a more feminine look that better aligns with their gender identity. They are often combined with other facial feminization procedures like forehead contouring and feminization rhinoplasty to achieve an optimal aesthetic outcome.

Facial implants are solid devices made from biocompatible materials like silicone that are surgically placed in areas like the chin, cheeks or jaw to enhance or reshape facial contours. For feminization, they are often used to create higher cheekbones or a more rounded chin. Benefits include:
- Permanent results. Once an implant has been placed, there is no need for repeat treatments.
- Customizable. Implants can be sculpted to achieve very specific shapes that are tailored to match the person’s unique facial anatomy and aesthetic goals.
- Significant structural change. For those with very angular bone structure, implants can soften sharp angles and create a more oval or heart-shaped face, often associated with femininity.
What Are Facial Fillers?
Facial fillers are substances that are injected into the face to add volume and smooth lines. For feminization, they are often used to plump the lips, enhance the cheeks or fill in under eye hollows. Non-surgical and temporary, fillers last 6 – 24 months depending on the product used. Benefits include:
- Nonsurgical and minimally invasive. Fillers are usually a quick office procedure typically taking 15 – 60 minutes with minimal downtime.
- Temporary and adjustable. The use of fillers allows flexibility to try different looks or adjust over time if results aren’t satisfactory.
- Low risk of complications. Side effects are usually mild and may include temporary swelling, bruising or redness.
Fillers or Implants? Making the Decision
Each option comes with unique advantages and limitations and the procedure(s) you choose to undergo will depend on your aesthetic goals, budget and willingness to undergo invasive procedures. Things to consider include:
- Cost. Although fillers come with lower upfront costs, repeat treatments are required to maintain results, which can be expensive over time. In comparison, surgical implants require a one-time, more expensive cost but offer permanent results.
- Acceptable risk level. Although the surgery is very safe, it does require general anesthesia which always carries some risk. There is also a possibility that the body will reject the implant, requiring removal or revision surgery. Fillers, in comparison, are noninvasive with minimal risk.
- Downtime. Recovery from implant surgery can take several months and involves swelling, bruising and some discomfort throughout the healing process. Filler procedures require no downtime and most people can return to their normal activities almost immediately.
