Rhinoplasty (nose job) is a procedure that is often performed to improve both the appearance and function of the nose. Feminization rhinoplasty involves similar techniques but is primarily intended to alter the nasal structure to appear more feminine. Often requested by transgender women, feminization rhinoplasty can be an important step toward achieving a softer, more feminine-looking nose to better align with their gender identity.
Feminization rhinoplasty is often combined with other facial feminization procedures to achieve an optimal aesthetic outcome.

All facial plastic surgery procedures are highly customized based on the person’s unique anatomy and expectations. Therefore, the first step is to schedule time with one of our plastic surgeons for an in-depth consultation. At this pre-planning appointment, the doctor will:
- Assess your nasal structure, often using imaging scans like computed tomography (CT).
- Use imaging software to visualize potential outcomes.
- Discuss additional, facial feminization procedures that may be recommended to achieve facial balance.
- Explain how the procedure will be done and what to expect during the recovery period.
Goals of Feminization Rhinoplasty
Certain nasal features such as larger overall size and a more angular, prominent bridge are often perceived as typically masculine. Therefore, the goals of feminization rhinoplasty may include one or more of the following:
- Remove or reshape cartilage and bone to reduce the overall width or length of the nose.
- Refine the nasal tip to be more upturned or delicately shaped.
- Adjust the nostrils to be smaller or narrower.
- Create a less prominent nasal bridge.
The Procedure
There are two main approaches that may be used in feminization rhinoplasty:
- Open rhinoplasty. A small incision is made in the tissue that divides the nostrils (columella), allowing the surgeon to lift the skin and access the nasal structure. This is often the preferred approach for complex cases as it offers greater access.
- Closed rhinoplasty. All incisions are made inside the nostrils leaving no visible scars and is more suitable for less complex adjustments.
Both approaches are performed with the patient under general anesthesia and can take 1 – 3 hours depending on complexity. Recovery usually takes 1 – 2 weeks for initial healing during which time there may be significant swelling and bruising. Most patients will be fitted with a nasal splint to wear for the first week after surgery.
Initial swelling typically fades in 1 – 2 weeks but it can take several months to a year for the final results to become apparent.
