Forehead and Eyebrow Bone Contouring
The forehead and browbone are prominent features of a typically masculine facial structure. For transgender women seeking a more feminine look, forehead and brow contouring can create a less pronounced brow ridge and a smoother, more rounded forehead that better aligns with their gender identity.
Forehead and brow contouring is often combined with other facial feminization procedures to achieve an optimal aesthetic outcome.

Find a UH Facial Plastic SurgeonGetting Started
All facial plastic surgery procedures are highly customized based on the person’s unique anatomy and expectations. Therefore, the first step is to schedule time with one of our plastic surgeons for an in-depth consultation. At this pre-planning appointment, the doctor will:
- Assess your facial bone structure and thickness using imaging scans like computed tomography (CT).
- Use imaging software to visualize potential outcomes.
- Discuss additional, complementary procedures such as hairline advancement or orbital rim contouring to soften angularity around the eyes if needed for facial balance.
- Explain how the procedure will be done and what to expect during the recovery period.
The Procedure
The most common approach for forehead and brow bone contouring is to make an incision along the hairline or within the scalp to access the bones involved. The bones are then carefully shaved down or reshaped using specialized tools to create a smoother contour. A brow lift procedure may also be performed to elevate the eyebrows for a more feminine look.
In some cases, and if necessary for optimal results, forehead augmentation may also be performed using implants or fat grafting.
The procedure is performed under general anesthesia and generally takes 2-4 hours depending on complexity and other procedures that may be performed during the same session.
Once you are discharged, it is very important to follow all post-operative instructions exactly. In addition to recommendations for wound care, your doctor may advise you to:
- Avoid strenuous exercises and heavy lifting for at least 4 – 6 weeks.
- Take pain medications as prescribed
- Maintain a healthy diet and adequate hydration.
- Stay out of direct sunlight to minimize scarring.
- Schedule and attend all follow-up appointments with your surgeon.
When Will I See the Final Results?
Immediately after surgery, there will be swelling, bruising and mild discomfort around the forehead and eyes. A compression bandage may be used to minimize swelling and the head should be kept elevated as much as possible during the initial recovery period.
Swelling and bruising typically subside within 2 – 3 weeks and patients can usually return to light activities. However, long-term healing and final results can take from six months to a year. All incisions are strategically placed to hide the scars but even those that are visible will fade over time with proper care.
Risks of Forehead and Brow Bone Surgery
As with any surgical procedure, there are some risks to consider:
- Infection. This is rare but possible, particularly if postop wound care instructions are not followed.
- Nerve damage. Temporary numbness or changes in sensation around the forehead may occur but typically resolve over time.
- Minor facial asymmetry. Although unusual when the surgery is performed by a skilled surgeon, there may be some imbalance in the facial structure once healing is complete. This can often be corrected with a subsequent procedure or procedures.
