Membership will be confirmed once the enrollment document has been reviewed and payment has been processed.

Please consult your tax preparer for advice regarding the tax treatment of any expenses related to your medical care, including the UH Select annual membership fee.

Patients may cancel their membership at any time, however the membership fee is nonrefundable.

The following services and amenities are included with your annual membership fee. Additional office visits, specialty care and diagnostic testing are provided as necessary for an additional cost/co-pay and your insurance will be billed. If you do not have insurance, you will be billed directly.

• Easy access to same- or next-day appointments
• Preventive care, including chronic disease management
• 24/7 patient navigator access
• Direct physician email access
• Complete nutrition assessment by a registered dietician
• Comprehensive fitness evaluation
• Access to integrative health services including massage therapy and acupuncture

Your UH Select membership does not include emergency care. If you experience a life-threatening medical emergency, call 9-1-1 immediately.

Yes, members have direct email access to their UH Select provider. However, if you need an immediate response, we recommend you call us at 216-844-4YOU.

In the unlikely event that your physician is unavailable, your care will be provided by another physician within the UH Select group. All of our providers share the same level of commitment to your continued good health and have access to your secure electronic medical record (MyChart) to ensure continuity of care.

Your UH Select physician will take all the time that is needed to care for you and answer your questions. A visit may be as short as 15 minutes or as long as an hour or more.

If your medical situation is an emergency, call 9-1-1. For non-emergent issues, call us at 216-844-4YOU. We will do our best to facilitate the care you need, wherever you may be.

UH physicians are only able to share medical information with the patient and persons listed on the medical release form. If your loved one wishes you to have access to their information, they will need to complete and sign the appropriate documentation. Your provider can assist with this.

Yes, the program accepts all types of insurance, including Medicare. Please call the UH Insurance Access Line at 844-297-8817 with questions about your coverage and benefits. The hours are Monday through Friday, 8 a.m. – 6 p.m.

No, the annual fee is not billed to or covered by insurance. It is an out-of-pocket expense that covers all the enhanced services and amenities included with membership.

Yes, any additional office visits that are outside of the services covered under your membership will be billed to your insurance and a co-pay will be expected at the time of service. If you do not have insurance, you will be billed directly.

Yes, FSA and HSA funds may be used to cover co-pays and any other out-of-pocket expenses, with the exception of the annual membership fee.
