Lower Jaw Contouring
The lower jaw or mandible is a defining feature of the face, with a wide, prominent or angular jaw often associated with a typically masculine appearance. For transgender women seeking a more feminine look, lower jaw contouring can soften and reshape the jawline to better align with their gender identity.
Mandible contouring is often combined with other facial feminization procedures to achieve an optimal aesthetic outcome.

All facial plastic surgery procedures are highly customized based on the person’s unique anatomy and expectations. Therefore, the first step is to schedule time with one of our plastic surgeons for an in-depth consultation. At this pre-planning appointment, the doctor will:
- Assess your jawline structure, often using imaging scans like computed tomography (CT).
- Use imaging software to visualize potential outcomes.
- Discuss additional, facial feminization procedures that may be recommended to achieve facial balance.
- Explain how the procedure will be done and what to expect during the recovery period.
The Goals of Lower Jaw Contouring
The primary goal of facial feminization procedures is always to create facial balance while preserving function. Mandible contouring procedures typically strive to do one or more of the following:
- Reduce jaw width.
- Adjust the chin’s shape or size to complement the jawline and create facial balance.
- Soften sharp or square jaw angles for a more rounded appearance.
In some cases, a masseter muscle reduction procedure may also be recommended to further slim the jawline.
The Procedure
With the patient under general anesthesia, the surgeon accesses the mandible through incisions inside the mouth to avoid visible scarring. The jaw bones are then shaved or sculpted to reduce their width and angularity. The procedure typically lasts 2 – 4 hours depending on complexity
Patients can expect to experience significant swelling and discomfort for 1 – 2 weeks after surgery. A soft-food diet and strict oral hygiene are recommended during this time to support healing.
Most people can resume normal activities in 2 – 3 weeks but it can take several months to a year for the final results to become apparent.
