Patient Information
We see patients by appointment only. We offer patients two convenient ways to schedule an appointment.
Patients can call our office directly at 330-954-7210 to speak with a receptionist, who will arrange an appointment with the provider of your choice, or patients can visit our Meet the Team page to book an appointment online, 24 hours a day.
Payment Policy
We accept most forms of insurance. Please bring your insurance card and driver’s license to each visit to ensure we have the correct information. All copayments are expected at the time of service. UH no longer accepts cash or check as in-person payment for services. You can pay by card (credit, debit, HSA, FSA, etc.), Apple Pay or Google Pay. You can also use UH MyChart to pay your bill online or before your appointment during eCheck-in. We accept checks for payment by mail.

- Prescription Policy

Our providers are unable to prescribe new medications or change medications without seeing you first. A provider must review and approve all prescriptions during an appointment.
- Prescription Refill Policy

Please request refills in advance to ensure you don’t run out of medication. Refills can be requested in your UH MyChart account or by calling our office. Some medications may require an appointment before we can approve a refill. If your pharmacy of choice has changed, be sure to let our office know before your refill.
- Form Policy

If you need a form completed for school, work, or other reasons, please bring it to your next appointment. If you do not have an upcoming appointment, contact our office during normal business hours to discuss. Some forms may require an office visit, and a processing fee may apply depending on the type of form.
- Work/School Note Policy

Providers are unable to give notes without seeing you or for past dates if you were not seen in our office. If you need a provider note to give an excuse for missing work or school, or to clear you to return to work or school, please discuss with your provider during your visit or contact our office.
Excuse notes will only include the dates and details discussed during your visit. If you need more time off, a follow-up appointment may be required.

Same-Day Sick Care
At University Hospitals, we believe good health starts with great primary care, and that should be your first choice for care. However, there are times when a virtual or urgent care visit may be the better option for same-day care for minor illnesses and injuries.
