Co-Trigger: Lupron and Pregnyl/HCG Patient Instructions
Lupron (Leuprolide) Co-Trigger with 1500 IU Pregnyl (HCG) – SUBQ
Special Considerations: Please disregard the instructions on the box of HCG 10,000IU. The medication should be administered as follows. Both Lupron and HCG Trigger will be administered subcutaneously into the lower belly area.
Storage: Store both Lupron and HCG at room temperature until ready to trigger. Once Lupron vial is punctured and used to trigger there will be extra medication left, discard any remaining medication after 30 days. Extra mixed Pregnyl solution is stable when refrigerated 60 days.
Supplies
Lupron: BD insulin syringe microfine 28 gauge x ½ inch 1ml to draw up and inject trigger, alcohol swabs.
HCG: 3 mL syringe to mix HCG Trigger, 22 gauge 1½ inch needle to mix HCG Trigger, 1 mL syringe to draw up and administer HCG Trigger, 27 gauge ½ inch needle to inject HCG Trigger, alcohol swabs.
General Instructions
- Your dose of HCG will be 1,500 IU – that is equal to 0.15 mL on the 1 mL syringe.
- Your dose of Lupron will be 40 units in the insulin syringe.
- You will inject the Lupron immediately after injecting the HCG. This is to be done at the exact time as instructed by your nurse.
Instructions for HCG Trigger: Subcutaneous
- Wash your hands and work on a clean, dry surface.
- Tighten the 22 gauge 1 ½ inch needle onto the 3 mL syringe.
- Pull back on the plunger of the syringe to fill the syringe with 1 mL of air.
- Inject the 1 mL of air into the vial of bacteriostatic water. Hold the vial upside down and withdraw 1 mL of bacteriostatic water.
- Inject the 1 mL of bacteriostatic water into the vial of powder. Withdraw the needle and syringe from the vial and discard them.
- Gently swirl around the contents of the vial until the powder is dissolved.
- Using the 1 mL syringe and 27 gauge ½ inch needle – needle- you will now draw up 0.15 mL of the mixed medication in the vial for your dose (0.15 mL = 1,500 units).
- Place to the side until it is time to inject.
Instructions for Lupron Trigger: Subcutaneous
- Use the insulin syringe to draw up straight Lupron from the vial for your dose.
- Your dose of Lupron will be 40 units.
Administration

- Choose an injection site on your abdomen (see image).
- Clean the area with an alcohol swab and allow to dry.
- Insert the needle straight into the skin – do not go in at an angle.
- Push the plunger all the way down, then wait 5 seconds before removing the needle from the skin.
* At the exact time you were previously instructed, inject the 0.15 mL HCG under the skin in lower abdomen.
** Immediately after injecting the HCG, inject the 40 units of Lupron trigger under the skin in lower abdomen.
