Our health coaches create a supportive environment where you are the expert. You know what is best for you based on your personal priorities and values. Working with a health coach, you can expect to have a patient-driven experience. Together, you’ll create your personal Vision of Well-Being – the framework for your lifestyle plan. You’ll set small, achievable goals while receiving the support you need to address obstacles that arise, adjusting your plan as you go to make it better fit your life.

Your coach is your ally in getting from where you are to where you want to be. They will listen and ask questions to help you stay on track and make progress. Ultimately, with your coach’s help, you’ll learn to recognize your strengths and how to use them to reach your goals. You’ll also learn to believe in your abilities and have the confidence to make your desired changes.

Our health coaches not only help patients identify and prioritize personal lifestyle changes, but also incorporate many supportive therapies into their health journey.

Health coaches at University Hospitals are integrated throughout the health system, allowing them to work in collaboration with a patient’s entire medical team. With health coaching, patients receive individualized care and personalized help that integrates clinical recommendations.


Group Coaching

At University Hospitals, health coaching appointments are offered in individual or group formats, virtually or in person, depending on the patient’s preference.

While individual coaching allows in-depth time for the patient and their coach to strategize around their individual needs, group coaching can offer the same incredible support with the added benefit of social connection. Among other benefits, group coaching:

  • Helps patients feel less isolated in their health journeys.
  • Provides a confidential space for those in similar circumstances to learn from each other.
  • Empowers and encourages patients to continue working toward their health goals.

UH Health Coaching Pathways

Health coaching at UH Connor and UH Primary Care is a structured program delivered by a board-certified health coach and designed to help you achieve your health goals. One complete pathway consists of eight coaching sessions over the course of 12 weeks. The first session is 60 minutes long, followed by seven 30-minute sessions over three months. You will be asked to complete surveys through your UH MyChart to help document your lifestyle behavior changes, helping both you and your coach keep track of your progress.

During the first health coaching session, your health coach will help you prepare for your well-being journey and select one of four pathways related to our UH Pillars of Well-Being. The four pathways to choose from are:

  1. Move: physical activity, functional movement and reducing sedentary lifestyles
  2. Nourish: diet quality, culinary skills, meal preparation and mindful eating
  3. Connect: quality relationships, belonging, purpose and community
  4. Refresh: sleep quality, stress management, self-care and resilience

Research shows that people are more likely to be successful when they focus on one major lifestyle area at a time. Success is also more likely if you continue to meet with your coach for at least six months, so after completing one cycle of the program, we recommend beginning a second pathway to continue your well-being journey.


UH Health Coaching Payment & Billing

Currently, most University Hospitals health coaching services are self-pay.

University Hospitals health coaching is a qualifying expense for most Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA).

Some health coaching services may be a part of provider visits billed through insurance, such as group medical visits (also known as shared medical appointments) or as part of collaborative care visits. Please check with your healthcare provider to understand if your health coaching services fall within these categories.
