Advanced Pharmacy Practice Experiences at UH Samaritan Medical Center
University Hospitals Samaritan Medical Center is a 55-staffed-bed acute care facility located between Cleveland and Columbus. UH Samaritan Medical Center is the only hospital in Ashland County and also serves Mansfield and Loudonville areas. The main hospital facility includes a 24/7 emergency department, a medical/surgical unit, an intensive care unit, and a level II cath lab. UH Samaritan Medical Center offers a variety of inpatient and outpatient services.
Inpatient pharmacy services include medication procurement, storage, repackaging, dispensing, distribution, controlled substance services, sterile product preparation, drug information, drug interaction, medication reconciliation, monitoring of drug therapy, and clinical rounding with physicians and nurses. Omnicell automated dispensing cabinets are utilized for medication storage and inventory control in the pharmacy and on the nursing units. Ambulatory pharmacy services include anticoagulation clinic, infusion center focusing on oncology/hematology services, and retail pharmacy.

John Scott, PharmD, BCPSInternal medicine is a 1-month advanced pharmacy practice experience at University Hospitals Samaritan Medical Center. UH Samaritan Medical Center has a twenty-bed medical/surgical unit and an eight-bed intensive care unit (ICU). This rotation will focus on rounding and making recommendations for patients on the medical/surgical unit and ICU. The student will be responsible for participating in triad rounding with one of the two hospitalist teams every morning along with the pharmacist preceptor. Triad rounding consists of bedside rounding on the patient’s room with the hospitalist, nurse, and pharmacist. Prior to rounding, the student will be responsible for analyzing patients for the perspective hospitalist team and develop pertinent recommendations. The student is expected to evaluate the following, but is not limited to the following: current medications, lab values, imaging, chief complaint, medical history, etc. The student will discuss recommendations with pharmacist preceptor prior to attending rounds to determine appropriateness of recommendations. After rounding, the student will be expected to document recommendations as i-Vents in the patient’s chart and update the summary in the handoff section in the patient’s chart. The APPE student will also participate in medication education for inpatients regarding current therapy.

Ryan Meikle, RPhTransitions of Care (TOC) is a 1-month advanced pharmacy practice experience at University Hospitals Samaritan Medical Center. UH Samaritan Medical Center has a twenty bed medical/surgical unit and an eight bed intensive care unit (ICU). This rotation will focus on medication education for patients discharged from the hospital on both med surg and the ICU. The student will work closely with the discharge nurse, charge nurse, social workers, hospitalist, nurse practitioner (NP) or physician assistant (PA), physical therapy, and other healthcare professionals involved in the patients care. The student will review discharge medications and the discharge summary to ensure the discharge medications are appropriate. The student will work with the TOC pharmacist regarding any concerns or recommendations. Recommendations will be made to the appropriate provider. APPE student may assist with medication affordability at the discretion of the provider and schedule follow up appointments in the anticoagulation and diabetes clinics as needed. If the patient being discharged would like their medication filled at UH Samaritan Retail Pharmacy, the student will participate in Meds to Beds.

Ben Hobson, PharmD, BCPSAnticoagulation is a 1-month advanced pharmacy practice experience at University Hospitals Samaritan Medical Center. This learning experience focuses on anticoagulation management in the outpatient setting. This clinic is fully managed by pharmacists trained to initiate and change anticoagulants and anti-diabetic therapy in collaboration with outpatient providers including physicians, nurse practitioners, and physician assistants. The student will get exposure to dosing, monitoring, and counseling patients on warfarin and DOAC’s.

Oncology/Hematolopgy is a 1-month advanced pharmacy practice experience at University Hospitals Samaritan Medical Center. This rotation will focus on chemotherapy, immunotherapy, and other intravenous infusions for various disease states. The student will be responsible for checking the drug, dose, frequency, duration, and diagnosis related to chemotherapy/immunotherapy regimens with oversight from oncology pharmacist. Additionally, the student will perform the final check on compounded chemotherapy, immunotherapy, and hematologic medications with oversight by the oncology pharmacist. The student will also assist with inventory and ordering of non-hazardous and hazardous drugs. There will be opportunities to provide education to patients, pharmacy staff, and/or nursing regarding oncology/hematology topics as determined by the student and preceptor.

Ryan Meikle, RPhHospital/General Medicine is a 1-month advanced pharmacy practice experience at University Hospitals Samaritan Medical Center. This rotation focuses on inpatient hospital pharmacy tasks including order verification, medication reconciliation, order reconciliation, sterile IV compounding, and clinical duties. Pharmacist clinical duties include but are not limited to: vancomycin pharmacy to dose, warfarin pharmacy to dose, renal dosing, IV to PO conversion, anticoagulant review, duplicate PRN review, and antimicrobial stewardship. APPE student will get exposure to inpatient hospital pharmacy by practicing sterile IV compounding and medication reconciliation with a technician, order verification and order reconciliation with a pharmacist. The student will be expected to review the clinical monitoring patient list, renal dosing report, and IV to PO report in EPIC for patients on the medical/surgical unit and ICU. The student will develop therapeutic recommendations after patient review and discuss with preceptor for appropriateness.
